Job Description

We are seeking a dedicated team member to join the Care Coordination department. This role functions as a hybrid between front desk and consumer support staff. The position serves as a central point of contact for phone calls, texts, emails, and internal communication—ensuring smooth operations and exceptional service for caregivers and consumers.

Key Responsibilities
Administrative and Front Desk Duties:
Greet and assist visitors (caregivers, nurses, and guests) in a friendly and professional manner
Answer and route incoming calls; assist with general inquiries
Monitor, respond to, and forward emails, texts, and voicemails as needed
Assist with office supply inventory and submit weekly supply requests
Coordinate mailing tasks, including greeting cards and outbound mail
Document actions and communications in agency systems
Submit weekly updates on completed tasks and challenges to supervisors

Consumer Care Coordination Support:
Support Care Coordinators with tasks related to matching caregivers with consumers’ service needs
Respond to urgent matters involving clients or caregivers in a timely and professional manner
Prepare weekly utilization and open shift reports as assigned by leadership.
Handle incoming referrals following supervisor instructions.
Provide guidance and support to other Consumer Care Assistants as directed by the supervisor.

Qualifications:
High School Diploma or equivalent (required)
Bilingual (English/Spanish)
Strong work ethic and goal-driven mindset
Comfortable using smartphones, mobile apps, and basic office software
