Overview
The Episode Care Coordinator RN is responsible for overseeing the coordination and management of patient care throughout their healthcare journey, ensuring optimal outcomes and efficient use of resources. This role involves working closely with patients, families, healthcare providers, and insurance companies to facilitate care transitions, develop care plans, and monitor patient progress across episodes of care.
Education
Associate's degree
Certification/Licensure
RN License (Required) - Active in Virginia and North Carolina
BLS (Required within 90 days)
Experience
2 years of nursing experience with Home Health (within the past 5 years)
